Short Term Rental Permit Application and Reporting Forms

Before you can operate a Short Term Rental within Monona, you must first obtain a Short Term Rental Permit. Permits must be renewed by June 30 each year. 

All forms should be submitted to the City Clerk.

Required Application Forms

The following forms must be submitted with all applications.

• STR-1: Property and Operator Information
• STR-2: Floor Plans
• STR-3: Quarterly Reporting
  • Quarterly Reporting forms for the previous quarter are due to the City Clerk by Feb 1, May 1, Aug 1, and Nov 1.
• STR-4: Guest Registry
• STR-5: Residence Affidavit
• Room Tax Form
• A copy of your Short Term Rental license from Public Health Madison and Dane County

Other Application Forms

The following forms should be submitted only if applicable to your property.

• STR-6: Condominium Association Permission
  • Complete if your property is part of a Condominium Association
• STR-7: Tenant Notification to Property Owner
  • Complete if you are a renter and attached a signed copy of your lease
